According to the compounding compliance policy guide (CPG), the section about Compounding of Drugs for Use in Animals, a convincing relationship is compulsory when compounded medicine is prescribed.</p>

When well-controlled clinical studies are not available, then we must use more unsubstantiated supportive information which includes data collected from other species, pharmacokinetic data, case series and/or anecdotal reports. As the specificity and impartial nature of the supporting data weakens, our confidence in the drug&#8217;s result should also weaken. We must keep in mind the value of the available evidence for safety and effectiveness, when applying any remedy.<span id="more-36"></span><br />
Equine veterinarians are quite familiar, and to some extent comfortable, with the use of drugs in the absence of any scientific evidence. The literature is full of case series, case reports, pharmacokinetic and in vitro data (microbiology) information, but controlled prospective clinical studies are exceptional.</p>
<p>With all of this ambiguity, the least we must know is that we are actually administering the drug that we intend to administer. This is innate for drugs approved by many government regulatory agencies (e.g., Health Canada, United States Food and Drug Administration). Regulatory agency approved preparations have high quality of manufacturing controls and maintain a high level of confidence regarding product content. The dilemma of equine veterinary medicine is that practitioners find themselves confirming to conditions for which there are no clinical data to support therapeutic choices.</p>
<p>Equine practitioners must treat animals when no approved formulation exists, or where approved formulations are not viable to administer. In these situations, equine veterinarians frequently use compounded medications.</p>
<p>Drugs are administered with the anticipation of achieving a desirable effect. Veterinarians often find themselves in situations where the only choice for treatment is a compounded medication. However, use of compounded medications is restricted ethically and scientifically by several provisions that include, but are not necessarily limited to, the development of a veterinarian-client-patient relationship, prescription drug dispensing restrictions, and the formulation&#8217;s use in the treatment of disease, but not to improve performance. Besides, there should be no government regulatory agency (e.g., Health Canada, United States Food and Drug Administration) approved product in an appropriate dosage form to treat the condition.</p>
<p>With a view to achieve a positive effect when administering drugs, it is very basic to know that the drug is administered in the desired strength and that it is duly available to the patient.</p>
<p>If practitioners choose to compound, or to prescribe compounded medications, they take the responsibility for formulation effectiveness, safety and composition. Approved drugs are always formulated to specification. Guarantee of a formulation&#8217;s composition or its availability to the patient is relatively less when veterinarians prescribe compounded medications. Therefore, it’s necessary that veterinarian ask some specific questions before choosing a firm to compound a prescribed formulation.</p>

How can veterinarians avoid being affected by gossips of adulterated and contaminated food and drugs and still maintain the public’s trust?</p>
<p>Dawn Merton Boothe, DVM, Ph.D., Dipl. ACVIM and ACVCP, director of pharmacology at Auburn University, asked veterinarians to contest the adulteration of products in her letter to the editor in the Dec. 1 Journal of the American Veterinary Medical Assn.<br />
Compounded drugs aren’t approved by the Food and Drug Administration. When approved drugs are not available, or are not suitable because of the way they are packaged by the manufacturer for a doctor’s intended use in a particular patient, compounding of that drug is allowed. Though the compounding industry is growing fast, it is not well regulated yet.<span id="more-38"></span></p>
<p>Veterinary compounding is regulated by 50 state boards of pharmacy that use unclear federal laws that are inferred inconsistently. Most compounding pharmacies are run by pharmacists of undependable expertise who keep an eye on the preparation of the ingredients.</p>
<p>Veterinarians must understand that compounded drugs offer no guarantee of potency, stability, bioavailability, safety, or effectiveness. We have become relaxed using FDA-approved products, with guaranteed analysis, efficiency and warnings regarding unfavorable events while using that particular drug as a single agent or in combinations with other drugs.</p>
<p>The hundreds of compounding pharmacies all over the country do not use uniform methods or bases for their compounded products. Only a small number of pharmacies run tests on the stability of their acquired and outsourced compounds.</p>
<p>Some are unlawfully manufacturing medications under the disguise of pharmacy compounding. The field has spread out into a confusing crossover situation for veterinarians. We have human pharmacists formulating products for animals. If they do not respect and know the human-animal bond, they may purchase “bargain” bulk or raw chemical formulations made in China and other untrustworthy sources to save money. Without testing these bargain agents for consistency, stability, and impurities, our patients are at risk.</p>
<p>For instance, potassium bromide, cisapride, diethylstilbestrol, and other discontinued or no-longer-approved human drugs can be formulated only from bulk chemicals for veterinary medicine, and they are slackly supervised. We need to protect our veterinary patients from being maltreated by this potential danger.</p>

Further, it is the veterinarian&#8217;s responsibility to practice medicine scientifically as well as ethically.</p>
<p>If we believe these statements to be true and valid, then we can understand that various levels of scientific or empirical evidence regarding the nature of a particular drug formulation provide us with different levels of assurance that patients will respond positively to the administration of a drug. The highest assurance for safe and effective use of a drug comes when the available drug formulation that includes the dosage form, excipients, and concentration, etc. which was associated with a sought-after response, without troublesome adverse effects, as evidenced in controlled clinical tests that evaluated safety and efficacy.<span id="more-40"></span></p>
<p>When well-controlled clinical studies are missing, we must use more questionable supportive information which includes pharmacokinetic data, case series and/or anecdotal reports, and data collected from other species. As the specificity and balanced nature of the supporting data weakens, our confidence in the drug&#8217;s effect should lessen. We must keep the value of the available facts for safety and effectiveness in mind when applying any therapeutic. Equine veterinarians are somewhat familiar, and to some degree comfortable with the use of drugs when the scientific evidence is lacking.</p>
<p>The literature is full of case series, case reports, pharmacokinetic and in vitro data (microbiology) information, but controlled prospective clinical studies are exceptional</p>
<p>With all of this ambiguity, the least we must be on familiar terms with is that we are actually administering the drug that we intend to administer. This is natural for drugs approved by many government regulatory agencies (e.g., Health Canada, United States Food and Drug Administration). Regulatory agency approved formulations have to pass a high quality of manufacturing controls and support a high level of confidence regarding product content.</p>
<p>The problem with equine veterinary medicine is that practitioners find themselves attending to conditions for which there are practically no clinical data to support therapeutic choices. Equine practitioners should time and again treat animals where no approved formulation exists, or where approved formulations are impossible to administer.</p>
<p>In situations like these, equine veterinarians often use compounded medications.</p>
<p>Drugs are administered in the anticipation of achieving a desirable effect. Veterinarians often find themselves in situations where the only alternative for treatment is a compounded medication. However, use of compounded medications is restricted ethically and scientifically by several stipulations that but are not limited to, but include the development of a veterinarian-client-patient relationship, prescription drug dispensing restrictions, and the formulation&#8217;s use in the treatment of disease or to progress in terms of welfare, but not to improve performance.</p>
<p>Besides, there should be no government regulatory agency (e.g., Health Canada, United States Food and Drug Administration) approved product in an appropriate dosage form to treat the condition.</p>
<p>Something that is basic to achieving a positive effect when administering drugs is in knowing that the drug is administered in the required strength and that the drug is accessible to the patient.</p>
<p>If practitioners choose to compound, or to prescribe compounded medications, they assume the responsibility for the effectiveness of formulation, its safety and composition. <p><img class="alignright" title="http://www.vetmed.vt.edu/vth/la/img/allergy.jpg" src="http://www.vetmed.vt.edu/vth/la/img/allergy.jpg" alt="" width="350" height="274" />Products approved for human use under the Medicines Act 1981 can be used as veterinary medicines only when prescribed by a veterinarian and used in accordance with this standard. This comprises of medicines, new medicines, pharmacy-only, prescription and restricted medicines and substances.</p>
<p>Human medicines have not been evaluated for safety or efficacy in animals, nor have they been appraised with respect to potential residues if used in food-producing animals. They would have been manufactured in accordance with good manufacturing practices but they may not have been manufactured in facilities approved by the ACVM Group.</p>
<p>They are not necessarily prepared or packaged appropriately for use as veterinary medicines, they could include forbidden substances (see www.nzfsa.govt.nz/acvm for current list), and their labelling is probably irrelevant to their use as veterinary medicines.<span id="more-42"></span></p>
<p>This therefore means that there is no information evaluated by the ACVM Group that endorses the use of human medicines as veterinary medicines. However, they may be used as such when a veterinarian accepts full responsibility for their use and is very careful with respect to managing the risks. Although they are not registered PAR products, but even then they may be used as veterinary medicines only if approved by a veterinarian.</p>
<p>Specially compounded veterinary medicines: Specially compounded veterinary medicines are either compounded by a veterinarian, or by a skilled and experienced manufacturer, or a contracted compounding pharmacist on behalf of that veterinarian. They must be prescribed for a particular clinical need recognized in a veterinary consultation for animals under the observation of that veterinarian. In fact, specially compounded veterinary medicines may be prescribed only by the veterinarian who directed compounding in the first place.</p>
<p>This class does not take account of preparations formulated by veterinarians for common sale or for sale to other veterinarians or traders. The latter products must be registered except when they are specifically exempt from the need to be registered. The authority to compound and the freedom from registration has been provided to make sure that veterinarians can address a particular clinical need. The provision must not be used by veterinarians to avoid the product registration process.</p>
<p><img class="alignleft" title="http://media-files.gather.com/images/d161/d701/d744/d224/d96/f3/full.jpg" src="http://media-files.gather.com/images/d161/d701/d744/d224/d96/f3/full.jpg" alt="" width="286" height="330" />This class does not take in the decanted or broken down proprietary veterinary medicines when there is no change in the product itself. These are a part of unregistered veterinary medicines.</p>
<p>The compounding specifications corresponding to the manufacturing specifications, such as preparation, packaging, and labelling, is according to the instructions of the veterinarian as specified in a compounding order issued by that veterinarian. If the specially compounded medicinal preparation is formulated by someone other than the veterinarian, the instructions must be detailed in that compounding order.</p>
<p>Like human medicines, specially compounded veterinary medicines are not evaluated by the ACVM Group, so there is no information appraised by the Group that endorses their use as veterinary medicines. Yet, they may be used per se when a veterinarian accepts full responsibility for their use and takes due care to deal with the risks. Even though they are not registered PAR products, they may be used as veterinary medicines only if prescribed by a veterinarian.</p>
<p>Unregistered veterinary medicine trade name products: There are several proprietary veterinary medicine trade name products registered in many other countries that are not registered in New Zealand. The accessibility of these products is promoted internationally and they can be bought overseas or ordered by electronic means or by email. However, they must not be imported without aforementioned approval of the ACVM Group.</p>
<p>Under normal circumstances, trade name products that are not registered under the Agricultural Compounds and Veterinary Medicines Act 1997, or particularly exempted from the requirement to be registered under the Agricultural Compounds, and Veterinary Medicines Regulations 2001 should not be used as veterinary medicines in New Zealand.</p>
<p>On the other hand, under very special circumstances approval for their use and, subsequently, approval to import will be issued by the ACVM Group. In this instance, approval will be issued only if there is an instant welfare need that cannot be successfully met by using a product that is currently approved in New Zealand.</p>
<p>This class also takes in decanted or broken down registered veterinary medicines for the reason that products that have been modified are no longer the registered products.</p>

<p>We can also prepare the medication using more than a few unique delivery systems, such as a sublingual troche or lozenge, a transdermal gel or sprays. Or for patients who find it difficult to swallow a capsule, syrup can be prepared.</p>
<p>As a pet owner you would want your pet to receive the highest quality of veterinary care and you must be well aware that animals can be extremely difficult to treat with medications. For example, dosages can be quite tricky in treating dogs due to their varying shapes and sizes.<span id="more-44"></span></p>
<p>When the pet refuses to take medication because of the taste, it is often a prime opportunity for compounding. Because all pets come in different shapes and sizes, not all commercially made drugs are appropriate for them. Also, sometimes a manufacturer might discontinue the production of a medication.</p>
<p>In all these types of situations your veterinarian can recommend you a tuna, liver or chicken flavored suspension, or a transdermal gel that can be applied to an ear with an amount that is just right for your pet’s size and condition.</p>
<p>Stanozolol is an anabolic steroid indicated as adjunctive treatment to improve appetite, help weight gain, and enhance strength and vitality in horses, dogs, and in some rare cases, in cats.　Winstrol-V (Stanozolol) was discontinued by the pharmaceutical manufacturer, and as such veterinarians may have difficulty in obtaining this product on a regular basis.</p>
<p>One of the most common neurological diseases in dogs is Epilepsy. Epilepsy basically refers to repeated seizures in dogs, and seizures are an indication　that your dog&#8217;s brain is not functioning well.</p>
<p>Dogs with epilepsy normally begin seizing at between one and three years of age, and certain breeds are prone to develop epilepsy. A few breeds have demonstrated hereditary epilepsy, while in most it is just a strong disbelief.</p>

<p>Once your veterinarian determines that your dog is indeed suffering from epilepsy, the next step will be to decide and work upon the treatment strategy. If the epilepsy is suggestive, sometimes treating the primary disease (for example, removing　a brain tumor) will cure the epilepsy. If the epilepsy is idiopathic, which means that the cause can not be determined, then medications will generally be prescribed to help control both the frequency and intensity of the seizures.</p>
<p>Phenobarbital is one of the most commonly used antiepileptic medications in dogs. Besides being used on a daily basis to prevent seizures, phenobarbital is regularly used to stop seizures in progress.</p>
<p>Potassium Bromide is one of the first antiepileptic drugs discovered for humans, and it has been a priceless drug for veterinarians when treating epilepsy in dogs.</p>
<p>It is important that phenobarbital and potassium bromide be given out in the following ways:</p>
<p>In a dosage form that is most suitable for you and your dog. These are medicines that you will be giving on a daily basis and the last thing you want is to add more stress to your life, or your dog&#8217;s.</p>
<p>In a potency that is specific to your dog&#8217;s age/weight/breed. A fixed dose pill may provide too much or too little medication since it is mass produced and may not meet your dog&#8217;s unique needs.</p>
<p>Select a flavor that will help in administering the medication easily.</p>

<p>But the question is why should one consider compounding as a solution for their pet&#8217;s medical problems? That can be better answered with another question: How difficult is it for you to get your cat to swallow a pill?</p>
<p>We know that animals can be extremely difficult to treat with medications. Cats are infamous for refusing to swallow a pill, and usually eat right around one that is disguised in food. And dosages can be very risky with dogs. A medication that works for a 3D-pound Golden Retriever may be far too much for a six-pound Yorkie to take.<span id="more-46"></span></p>
<p>Humans and their pets often have variations of the same diseases that include skin rashes, heart conditions, eye and ear infections, cancer and diabetes. Pet medications, however, pose unique problems that are often best handled through compounding.</p>
